Its use in identifying sexual preferences has been demonstrated throughout the more than 60 years that it has been used for this purpose. It is therefore considered the gold standard (the most effective assessment method) for identifying certain paraphilias, such as paedophilia or hebephilia. Although it is superior to other assessment procedures in terms of reliability and validity, its clinical use in identifying the risk of committing sexual offences or recidivism in this respect (and the multiple opportunities this provides in assessment and treatment in the field of sexual therapy) its use among European professionals – especially Spanish ones ¬ – is marginal. Among other reasons, its limited dissemination is explained by a shortage of materials for providing training in its use, which is sometimes difficult. This manual deals with the steps to follow in order to set up a plethysmographic assessment from a clinical and educational point of view.
